Cyst drainage set The cyst drainage set consists of a cystostome with an extended pusher and a 2-flap endoprosthesis. The cyst drainage set was developed for the endoscopic puncture with HF-current. The endoprosthesis for cyst drainage can be positioned without changing the instruments. Removable connections The puncture wire can be removed and replaced by a guide wire (0.035 inches). Application of the cyst drainage set Figure 1 Use of color doppler to exclude larger vessels in the stomach or cyst wall prior to the planned puncture. Endosonographic representation of a large, not quite anechoic free, pancreatic pseudocyst. Figure 2 Position of the cystostome after puncture of the cyst using HR-current. Figure 3: Corresponding image for figure 2 in the fluoroscopic control. Figure 4: If necessary, the cyst can be filled with a contrast agent. In the photographic situation, the puncture wire is replaced by a guide wire under radiologicalendosonographic control. This creates a more stable access to the cyst which allows the direct placement of the pre-loaded stent via the cystostome. Figure 5 The already pre-loaded stent can be easily pushed forward in the pseudocyst under endoscopic and / or radiological control after loosening the pusher. Similar to ERCP, the last flap respectively the second metal ring has yet to be identified in the stomach. Figure 6 Final radiological control: The two platinum rings allow the rapid identification of the stent position. Figure 7 Endoscopic control of the effective drainage. Figure 8 In the present case, greenish tinged liquid.
